Bilateral intraocular Hemorrhage Associated with Traumatic Intracranial Hematoma

|
A 48 year old male complained headache and floaters in the right eye after the recent fall down injury. Brain computed tomography (CT) and thorough ophthalmic examination were performed. A small amount of sub-dural and intracerebral hematoma was found in the brain CT. Fundus examination revealed bilateral papilledema, pre-retinal hemorrhage and vitreous hemorrhage. Most of the intraocular hemorrhage was originating from the peri-papillary area. The visual acuity, color perception and pupil light response were not affected until the complete absorption of the blood and the resolution of papilledema. The clear view of fundus in a bilateral Terson syndrome with minimal visual impairment enabled the clearer understanding of the mechanism and the origin of intraocular hemorrhage in Terson syndrome.
